How It Works

Easy as 1-2-3.

1 Sign the doctor’s order and our contract to enroll your patient for RPM by HealthConnect.

2 Your patient must record vitals and use their provided devices for 16 days every 30 days for Medicare reimbursement.

3 The RPM liaison will monitor vitals daily and let you and the patient know if anything is out of range. They will also provide advice on how to improve vitals and their condition.
