1 Talk to your doctor about enrolling to RPM by HealthConnect and getting in touch with us.
2 Record your vitals and use your provided devices for 16 days every 30 days for Medicare reimbursement.
3 Your RPM nurse will monitor your vitals daily and let you and your physician know if anything is out of range. They will also provide advice on how to improve your vitals.
